Bias-Related Incident Report


The University of North Carolina at Charlotte is committed to fostering a safe and welcoming campus community for all who have been a target or witness to a hate or bias incident. This online form may be used to make the University aware of these incidents.

Your trust is very important to us, and we will make every effort to respect your privacy. However, in certain circumstances we cannot guarantee anonymity.

If your concern is related to dating violence, domestic violence, sexual or gender-based harassment, sexual misconduct, stalking, and/or related retaliation, please complete the "Report Sexual and Interpersonal Misconduct" form so that the information is routed directly to the Title IX Office for review.
