Incident Reporting Form


Staff, faculty, students and other community members may use this form to report:

  • Alleged Student violations the Student Code of Conduct or other policies
  • Reports when you are concerned for the wellbeing of other students
  • Alleged reports of Sexual Misconduct, Harassment, Discrimination or Bias
  • Other information you may feel necessary to report

When completing this form, please include your name and title. Although anonymous submissions are accepted, this limits our ability to investigate if needed. If you have any questions regarding the filing of an incident report, please contact the Office of the Dean of Students at 210-999-8843 or email us at deanofstudents@trinity.edu. Each report submitted is routed directly to the Dean of Students and the Assistant Director for Parent Communications & Student Conduct. Each report is reviewed to determine next steps, if appropriate.

This form is for NON-EMERGENCY reporting. To file an official police report, please contact the Trinity University Police Department at 210-999-7070 or in person at 538 Kings Court, San Antonio, TX 78212.

Involved Parties

Please list the individuals involved (excluding yourself), including as many of the listed fields as you can provide. For non-students, please list a Drivers License number in the block labeled Student ID #, if available. If you have listed your information as the person submitting the report, you DO NOT need to include your information below.

Definition of Roles:
Respondent/Alleged: Student(s) or Student Group/Organization allged to have engage in conduct that violates University policy.
Complainant: Student(s) who are the complaining party regarding the allegation. Note: This person may ask to remain anonymous if requested (this may limit staff to follow up as appropriate).
Student of Concern: Student(s) who may need support dealing with a crisis and/or concern. A report solely for this type of concern may also be submitted through the Student Care Report
Victim: Student who has been impacted by alleged behaviors of the Respondent.
Witness: Person who is able to provide information about the incident through direct observation or other knowledge, but this person is not alleged to have engaged in potential violation of University Policy.

Details of occurrence

Please provide a detailed description of the incident/concern using specific concise, objective language. If you have listed an individual as an involved person, they should appear at least once in the narrative.
 
For behavioral reports, please provide as much detail as possible in describing the behaviors and activities that occurred. The use of direct quotes is encouraged if appropriate. 

For other reports, please include as much description as possible regarding the circumstances that resulted in the report.

All reports are submitted to the Office of the Dean of Students for appropriate resolution.
