Please provide detailed information that you are reporting involving a Saint Louis University (SLU) student . For all emergencies contact DPS at 314-977-3000. Please do not use this form to submit information regarding Faculty or Staff. For Faculty and Staff concerns, please report your concerns to the SLU Office of Compliance and Ethics.
Student records, including incident reports, are protected by FERPA (Family Educational Rights and Privacy Act), therefore any information regarding the outcome of this report will not be shared with you without the direct written consent of the student(s) involved. Any exception to this policy can be found in the Student Handbook.
Please use the following guidelines when completing the "Nature of the Report".
Community Standard Violation: alleged violation of the University's Community Standards (except Sexual Assault) (eg. Covid-19 non-compliance, underage drinking, drugs, inappropriate behavior, restricted items, etc.)
Behavioral/Emotional Concerns: Suicide Ideations, odd behavior, notification of depressed students, eating disorders, etc. Immediate threats of harm to self or others is an emergency and should be directed to the Department of Public Safety at 314-977-3000. The Behavioral Concerns Policies and Procedures can be found in the Student Handbook.
Bias Related Incident: behavior exhibited toward a student based because of the actual or perceived race, color, religion, national origin, gender, sexual orientation, gender identity, or disability. The Bias Incident Response Protocol can be found in the Student Handbook.
Care and Concern: Student has been injured, has an illness or a death in the family.
Mediation: Two or more students are requesting a trained professional staff member to sit down with them to facilitate a conversation between the students resulting in conflict resolution.
Title IX Referrals: This report is used to refer any kind of sexual harassment, sexual assault, dating violence, domestic violence, stalking, or any other matter to be referred to the Title IX Coordinator. The University's Title IX Sexual Harassment Policy can be found in the Student Handbook.
Other: Used to document a report that does not meet any other criteria listed above.
