Reporters can utilize this form to file a report on their own behalf or on behalf of an impacted party.
To fulfill a duty to report under the Non-Discrimination, Harassment & Sexual Misconduct Policy or the Youth Activities and Programs Policy, you must include your name and all known details about the incident, including name(s), date, location, and description.
Filling out this form does not necessarily initiate an investigation. A Civil Rights Intake Coordinator will contact individuals in need of resources with an explanation of rights and options.
