Bias Incident Report Form


REPORTING TO THE UNIVERSITY
As part of its mission, Gonzaga University is committed to the dignity of the human person, diversity, and intercultural competence. Grounded in Gonzaga's mission as well as its harassment and non-discrimination policies, the Bias Incident Assessment & Support
(BIAS) Team strives to foster a safe, hate-free environment where all community members can live, learn, and work in leadership and service for the common good. The BIAS Team's primary roles are (1) to support individuals and communities impacted by bias incidents and hate crimes and (2) to assess and make recommendations on the impact of bias incidents and hate crimes on Gonzaga's overall campus climate.

If this is an emergency or an incident that requires immediate response, please dial 911 or contact Campus Security and Public Safety at 509-313-2222.

To report a bias incident or hate crime, please complete this form. If you wish to have a member of the BIAS Team follow-up with you, please provide your contact information. Someone will follow up within two business days. If you mark the report as “Notification Only” or submit the report anonymously you will not receive a follow-up from the BIAS Team. Please note that there may be extenuating circumstances that may require us to follow-up with a “Notification Only” or anonymous report. Typically this would be if the incident is egregious, violates University policy, and/or there is an established pattern of behavior. In all cases we strive to be a “reporter-driven” BIAS Team and we will attempt to contact you prior to follow-up with any other(s) involved.
