Tell Someone - Discrimination


The Tell Someone online form allows all members of the Creighton Community to report harassment, discrimination, grievances or other concerns not aligned with our University Mission. The information users provide will be private and Creighton staff members who receive this form have signed a non-disclosure agreement, committing them to maintain the highest level of privacy.

By completing this form expect that you will be contacted for any follow-up information concerning this report.

This reporting form should NOT be used to report an emergency. If you need to report an emergency, please call Public Safety at (402) 280-2911.



For Students at the Phoenix Campus:

Background Information

Creighton University requires that all faculty and all staff with supervisory or leadership responsibilities or responsibilities related to student welfare report issues of sexual violence, discrimination, and discriminatory harassment to the Office of Title IX and Civil Rights Compliance  (see Mandatory Reporters Policy 2.1.26)

Whenever possible, please be sure the person disclosing information to you understands your obligation to report this information to the Office of Title IX and Civil Rights Compliance . If the individual does not know that you are reporting this incident, please indicate this on the form below. 

This form will be submitted to Title IX Coordinator in the Office of Title IX and Civil Rights Compliance . If you need immediate assistance, please contact the Department of Public Safety at 402-280-2911.
