COVID Public Health Behavior Report Form


COVID-19 county and state public health order apply to all Colorado State University grounds and campuses, and must be followed by faculty, staff, students, and visitors. This form is intended to report behavior that appears to violate the University's COVID public health guidance. In addition, Colorado State University COVID-19 policy requires adherence.

All students, faculty and staff will be required to follow state and county public health orders, guidance, protocols and University policies while on university grounds and in all buildings, including classrooms, laboratories, living, research, service spaces, and university public spaces. These include:

       •    Wearing masks or other appropriate face covering;
       •    Maintaining physical distancing of six feet from other individuals;
       •    Disinfecting study and work surfaces (desktops, tables, etc.) regularly;
       •    Staying home if they are experiencing COVID-19 symptoms, receive a positive COVID-19 test, or believe they have been exposed to COVID-19;
       •    Never gathering in groups without public health precautions and avoid large groups;
       •    All faculty, staff, and students must fill out the daily symptom checker to report symptoms, a possible exposure or a positive test.

Enforcement action can only be taken if a person or persons are identified and enough information is available to take enforcement action. All reports will be reviewed to determine an appropriate response.

Involved Parties
